Non-insulin dependent (type II) diabetes mellitus, whenever blood sugar levels cannot be controlled adequately by diet, physical exercise, or weight reduction alone.
DOSAGE AND ADMINISTRATION:
Normally a long term therapy which must be initiated and monitored by a physician: Usual initial dose 1mg; Usual maintenance dose: 1mg to 4mg daily, Maximum dose 8mg.
CONTRAINDICATIONS:
Insulin dependent (type I) diabetes mellitus, diabetes ketoacidosis, diabetic precoma or coma, hypersensitivity to Glimepiride or any of it’s excipients, other sulfonylureas or sulphonamides, serious hepatic or renal dysfunction, dialysis patients.
SPECIAL PRECAUTIONS:
Regular monitoring of glucose level in blood and urine. Milder or absent symptoms of hypoglycemia, e.g, in-patient with autonomic neuropathy or taking beta-blockers, clonidine, reserpine, guanithidine or other sympatholytic drugs. Temporary change over to insulin in exceptional stress situations (e.g, trauma, surgery, infections with fever).
PREGNANCY AND LACTATION:
Contraindicated.
ADVERSE EFFECTS:
Main adverse effects: Hypoglycemia as a result of blood glucose lowering effect of Glimepiride and additionally signs of adrenergic counter regulation, temporary visual impairment, gastrointestinal symptoms (e.g, nausea, vomiting, abdominal pain, diarrhea) elevated liver enzymes, thrombocytopenia, hemolytic anemia, erythrocytopenia, leucopenia agranulocytosis, pancytopenia, allergic or pseudoallergic reactions (e.g, itching, rashes, urticaria) including shock, allergic vasculitis, hyper sensitivity to light, decreased serum sodium.
INTERACTIONS:
Potentiation of blood sugar lowering effect of Glimepiride e.g, by insulin or other (oral) antidiabetics, ACE-inhibitors allopurinol, anabolic steroids, and male sex hormones, MAO inhibitors, miconazole, oxphnbutazone, para-aminosalicylic acid, pentoxyphyllin, phenylbutazone, probenecid, quinolones, salicyclates, sulfinpyrazine, sulfonamides, tetracycline, tritoqualine, trofosfamide. Weakening of the blood sugar lowering effect to Glimepiride by for e.g acetazolamide, barbiturates, corticosteroids, diazoxide, diuretics, epinephrine (adrenaline) and other sympathomimetic agents, glucagons, laxatives (after protracted use) nicotinic acid (in higher doses), estrogens, phenothiazine, phenytoin, rifampicin, thyroid hormones. Potentiation or weakening effect of the blood sugar lowering effect, H2 receptor antagonists, clonidine, reserpine, acute and chronic alcohol intake. Reduced or absent signs of adrenergic counter regulation, sympatholytic drugs (such as beta-blockers, clonidine, guanithidine, and reserpine). Decreased glucose tolerance or deterioration of metabolic control: beta-blockers. Glimepiride may either potentiate or weaken the effect of coumarin derivatives. Prescription only medicine.
